At the end of each survey, you will get a report that shows your healthy habits and some of the things you are doing that are not so healthy. You will also be able to read and download some materials that can help you improve your health.

The PHQ-9 is designed to help you and your health provider screen you for depression signs and symptoms. The SSI-SA is designed to help you and your health care provider screen you for potential substance use difficulties and to help you get the assistance that you need to become healthier.
If you feel that you may need help with either depression or drugs or alcohol, you can fill in these screening tools, print them out and review them with your healthcare professional. GOBHI urges you to not interpret the results yourself but take the screening tool to your healthcare provider to assist you in making the best decisions for your health and wellbeing.
