Community Health Development
Overview
The Community Health Development Team develops productive local partnerships with community-based organizations, healthcare systems, local governments, Oregon Health Plan (OHP) Members, Tribes and other local entities who address Social Determinants of Health and Health Equity in order to facilitate locally-driven health systems transformations.
Function
The Community Health Development team acts as consultants, supporting and guiding the Eastern Oregon Coordinated Care Organization (EOCCO) communities with their expertise to create solutions that are driven by local needs and goals.

Support the ongoing development, membership, and functions of effective Local Community Health Partnerships (LCHPs) and Community Advisory Council (CAC_

Provide support for Social Determinants of Health and Equity Spending Programs- SHARE, LCHP, CBIR, HRS, and HRSN- 1115 Waiver funds

Health Equity Plan implementation

Develop, implement, and submit a Community Health Assessment and Community Health Plan in partnership with various Moda and GOBHI teams

Develop and submit annual report for the Community Health Improvement Plan progress report and the CAC Demographic Report

Facilitate Community Health Assessments and identification of priorities

Tribal Engagement with the Confederated Tribes of the Umatilla Indian Reservation (CTUIR) and the Burns Paiute Tribe
