¿Qué es Wraparound?

Wraparound es un proceso intensivo de planificación en equipo que sigue una serie de pasos para ayudar a los niños, los adultos jóvenes y sus familias a lograr su visión familiar. Este proceso de planificación de la atención individualizada es un compromiso compartido entre profesionales, jóvenes, familias y apoyos naturales a los 10 principios de Wraparound para impulsar el proceso.

¿Qué son los sistemas de atención?

Systems of Care es un espectro de servicios y apoyos efectivos para niños, jóvenes y familias con o en riesgo de salud u otros desafíos y sus familias organizados en una red coordinada que construye asociaciones significativas con jóvenes y familias y aborda sus necesidades culturales y lingüísticas. para ayudarlos a funcionar mejor en todos los dominios de la vida. Esta es una infraestructura organizativa necesaria para superar en colaboración las barreras intersectoriales en los sistemas de atención infantil que los jóvenes, las familias y los proveedores de sistemas están experimentando a nivel local. Wraparound pone en funcionamiento los sistemas de atención a nivel de la práctica.

Data Dashboard

This dashboard combines data from Child Welfare, the Office of Developmental Disabilities Services, Oregon Health Authority and juvenile justice. Data displays are under development in consultation with the System of Care Advisory Council and state agencies.

Barrier Resolution

Eastern Oregon has a Barrier process to resolve issues locally. A barrier is a problem or block to receiving needed services and support.

Do you know a young person or their family who has had an issue while navigating systems or accessing services? If so, then you’ve come to the right place. You can submit a request for help with your barrier.

How does the Children’s System of Care review barriers?  ​

Once you submit a barrier submission form, Children’s System of Care members will share your barrier to the local Practice Level Workgroup (PLW) who will work together to resolve barriers. If unable to resolve at PLW then it is sent to the Local Executive Committee or the State System of Care Advisory Committee for Busting. If you are interested in attending a Children’s System of Care meeting in your community, please email Jphelps@gobhi.org

*Please note: Not all barriers will be addressed every month, and this process is not intended to address urgent situations or crisis situations. This form does not replace the formal grievance process that exists for providers and system partners. Please do not include Protected Health Information on this form.

Youth and Family Leadership Committee

The Systems of Care Youth and Family Leadership Committee is a committee made up of Youth and Family Representatives that come together to elevate ALL Youth and Family Voice. Together we identify barriers Youth and Families are experiencing and submit to Systems of Care. A barrier is “Anything standing between a youth and family and them being successful/sustainable.”
